• Conduct thorough assessments of patients' mental health needs, employing a range of evidence-based tools, including the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), to inform individualized treatment plans.
• Monitor and evaluate patient progress effectively, utilizing outcome measures, with consistent adjustments to interventions as needed to support optimal mental health.
• Maintain comprehensive treatment records and clinical documentation in alignment with regulatory standards, utilizing electronic health records (EHR) for accuracy and timeliness.
• Collaborate closely with patients and their support systems to develop tailored treatment plans that reflect their unique cultural backgrounds and specific mental health challenges.
• Coordinate referrals to additional services as required, becoming a vital link between patients and various healthcare providers.
• Engage in mentoring and educational activities for students, interns, and other healthcare professionals in your field, thereby contributing to the development of future mental health practitioners.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid State License as a Licensed Clinical Social Worker (LCSW).
• Proven experience working with patients in outpatient settings is preferred.

Experience working with oncology patients or those dealing with chronic illnesses would be advantageous but not mandatory. Familiarity with telehealth platforms and an aptitude for leveraging technology in therapeutic practices will enhance your effectiveness in this role. Furthermore, strong communication skills and a collaborative spirit are essential for building rapport with patients and colleagues alike.

We offer a competitive salary range of $90,000 to $105,000 per year, commensurate with experience and qualifications.
