InGenesis is currently seeking a Social Worker Case Manager to review and evaluate medical or behavioral eligibility regarding benefits. In this role, you will assess and monitor medical necessity, options and services required to support members in managing their health.

Job Duties

โ€ข Provide telephonic support for members with chronic conditions.

โ€ข Provide active case management (assess needs, develop action plans, monitor services).

โ€ข Ensure coverage for appropriate services.

โ€ข Make referrals to appropriate staff

โ€ข Participate in data collection and entry.

โ€ข Ensure accurate documentation of clinical information.

โ€ข Perform other duties as assigned.


Minimum Qualifications

โ€ข Current Clinical Social Worker license in the State of South Carolina.

โ€ข Associates degree in nursing or graduate from accredited nursing school or Master's Degree in Social Work

โ€ข 4+ years of clinical experience.
