Position Summary
The Home Health Registered Nurse - Triage is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. S/he triages all patient and facility calls after hours and may include facilitating new and existing home health referrals from in and out of system, after hours, and support for home health field staff.
Position Specific Responsibilities
  • Accepts and facilitates new referrals for both in and out of the system for home health and hospice patients.
  • Acts as a liaison to provide community education for our referral sources, patients and families.
  • Provides accurate and responsive service to all calls received during weekend hours.
  • Collaborates with physicians and obtains orders as needed to facilitate patient care.
  • Provides guidance and support to field staff during hours worked.
  • Assists in scheduling patient admissions, routine visits, and PRN visits to field staff (Nurses, Therapists, and Home Health Aides).
  • Monitors telehealth as needed.
  • Performs other duties as assigned.

This position requires every Saturday coverage, along with varying weekday evening hours.
Education and/or Experience
  • Associate or Bachelor's degree in Nursing required.
  • Two (2) years of experience as a registered nurse in a clinical care setting or home health preferred.

Skills
  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.

Certifications, Licenses, and Registrations
  • Active and unencumbered Multi-State Registered Nurse license in state(s) of employment required.
  • Alaska, California, and Michigan RN Licensure required
  • Able and willing to obtain additional RN Licensures if necessary
  • Current CPR certification required.
