This position offers the flexibility of remote work, allowing the Licensed Clinical Social Worker to conduct patient assessments and provide therapy sessions from a suitable home office or telehealth setting. You will be integrated into a collaborative team that includes physical therapists, occupational therapists, and speech-language pathologists, ensuring a holistic approach to our patients' care.

• Conduct comprehensive psychosocial assessments through patient interviews, direct observations, and detailed reviews of medical histories to understand the emotional, social, and environmental factors affecting recovery.
• Provide counseling and emotional support to patients and their families, identifying and addressing common adjustment issues that arise during the rehabilitation process.
• Collaborate closely with an interdisciplinary team to develop personalized treatment plans that incorporate patients' psychosocial needs into their overall rehabilitation strategy.
• Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance, to ensure a smoother transition back into their daily lives.
• Educate patients and families about available community resources, and proactively facilitate post-rehabilitation services to support long-term independence.
• Participate actively in discharge planning efforts, ensuring patients have the necessary support and resources during their transition to home or other healthcare settings.

Your work will primarily focus on individuals recovering from significant health challenges, including strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. You will be instrumental in addressing both the emotional and practical needs of these patients, fostering their recovery and integration back into the community.

• A Master's degree in Social Work from an accredited institution.
• Current, valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.
• A strong understanding of psychosocial dynamics related to adjustment, grief, and coping mechanisms.
• Exceptional communication skills and the ability to build trusting relationships with patients and their families.

• Experience working in a rehabilitation or medical setting, including familiarity with interdisciplinary client care models.
• Prior experience providing support to patients undergoing significant life transitions, especially in relation to physical disabilities.

The salary for this position ranges from USD 75,000 to 100,000 annually, commensurate with experience and qualifications. We also offer a sign-on bonus to enhance recruitment efforts.
