We're hiring a Physical Therapist for our Hospital At Home Department in Jacksonville, Florida. This is a PRN Days position which will provide patient home-based healthcare throughout the Jacksonville community. The schedule for this position will be at least 3 shifts per month, Monday - Friday with no weekend or holiday commitments.
This Physical Therapist position will provide in-home patient care support (commuting involved to patient homes within the Jacksonville metro area with mileage reimbursement). This Physical Therapist will also provide floating coverage support to Baptist Health acute care rehab units in coordination with Rehab Services leadership beyond in-home patient care appointments as scheduling permits. The ideal candidate would have PT experience in both acute care and home health settings. Candidates with experience and interest in program / new service development are preferred.
We are looking for a Physical Therapist to join our Rehab Services Team at Baptist Medical Center. This position requires a candidate with at least 1 year of recent experience working in an acute care setting, home health patient care experience, and experience providing direct orthopedic and neurological physical care. This is a PRN opportunity working day shift hours.
Job Responsibilities:
  • Evaluates, plans, directs and administers physical therapy treatment to patients.
  • Provides patient care to help improve or restore function and prevent disability following injury, disease or physical disability.
  • Oversees support staff. Participates in departmental operations and in performance improvement activities.
  • Responsible for remaining within budget guidelines.
  • Participates in mandatory education and licensure requirements.

Experience Requirements:
  • 1 or more year(s) of home health Physical Therapy Experience strongly preferred
  • 1 or more year(s) of acute care Physical Therapy Experience strongly preferred
  • Orthopedic Experience

Education & Credential Requirements:
  • Bachelor's degree (or higher) in Physical Therapy Required
  • Licensed Physical Therapist from FL Department of Health Required
  • Basic Life Support (BLS) certification from the American Heart Association Required
