Position Overview

Community Outreach Network is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our remote team. This full-time position will focus on delivering critical support and resources to individuals and families grappling with complex life challenges, particularly in the hospice care setting. Key Responsibilities
  • Conduct thorough assessments to evaluate clients’ social, emotional, and environmental conditions, enabling informed decision-making in the support process.
  • Develop and implement individualized service plans that incorporate specific goals, necessary interventions, and methods for monitoring progress over time.
  • Provide empathetic counseling and support for clients and their families as they face diverse and often challenging life events.
  • Utilize active listening to collaborate with clients on their personalized service plans and effectively connect them to appropriate community resources.
  • Offer immediate crisis intervention strategies, perform risk assessments, and assist in crafting safety plans when required.
  • Establish and nurture partnerships with community agencies and organizations to enhance service coordination and advocate persistently for client needs.
  • Engage in proactive community outreach efforts to raise awareness of SCHN’s services and promote resources available for diverse populations.
  • Regularly participate in case conferences, contribute to team meetings, and engage in ongoing training to continually elevate service quality.
Patient Population / Client Focus

Your role will primarily involve working with individuals and families within the hospice care setting. This demographic often faces significant emotional and psychological challenges, especially related to grief, loss, and health care decisions. Understanding the nuanced needs of this client base will be essential as you provide comprehensive support tailored to their unique circumstances.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • Possession of an active and unrestricted License as a Clinical Social Worker (LCSW) in Texas is mandatory.
  • Strong grounding in social and economic justice issues along with familiarity with community resources specific to hospice care.
  • Proven capacity for communication, cultural competence, and problem-solving within sensitive contexts is essential.
Preferred Experience
  • Prior experience within a social service agency, particularly within hospice or clinical settings, is highly preferred.
