• Conduct comprehensive psychosocial assessments, gathering in-depth information regarding clients' mental health issues, strengths, and available resources.
• Deliver therapeutic interventions through individual, family, and group therapy sessions while utilizing established evidence-based treatment modalities.
• Implement evidence-based therapies including cognitive-behavioral therapy (CBT), solution-focused short-term therapy, and trauma-informed care approaches.
• Design and execute individualized treatment plans that reflect clients' specific goals and measurable outcomes.
• Collaborate seamlessly with psychiatrists, psychologists, and other healthcare professionals to ensure a cohesive and coordinated care experience for clients.
• Facilitate crisis interventions, conduct risk assessments, and develop safety planning as necessary to ensure client well-being.
• Maintain accurate and timely documentation in full compliance with ethical and legal standards, utilizing electronic health record (EHR) systems efficiently.
• Lead group therapy sessions, psychoeducation classes, and support groups to foster community and shared healing among clients.

Your practice will primarily focus on individuals receiving hospice care, who may experience a range of emotional and psychological challenges as they face end-of-life issues. You will work with clients across the age spectrum, addressing their mental health needs with compassion and expertise, assisting not only the patients but also their families to navigate complex feelings such as grief, fear, and anxiety surrounding death and dying.

• Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required.
• Minimum of two years of relevant experience in mental health, preferably within an outpatient or hospice setting.
• Strong understanding of evidence-based therapeutic modalities and strategies for mental health treatment.
• Exemplary interpersonal, communication, and assessment skills, with a focus on client-centered care.

• Experience working with end-of-life issues and hospice care, particularly in a remote setting.
• Adeptness with telehealth technologies and electronic health record (EHR) systems.
• Proficiency in group facilitation and psychoeducation.
