• Conduct thorough assessments and evaluations of clients to determine their mental health needs, social determinants affecting their care, and establish individualized treatment goals that align with hospice paradigms.
• Provide empathetic one-on-one counseling sessions to clients facing a variety of mental health and psychosocial challenges, tailoring interventions to meet their unique situations.
• Facilitate engaging group therapy sessions aimed at addressing common mental health concerns while enhancing support networks and delivering psychoeducation about grief and loss.
• Develop, implement, and periodically review personalized treatment plans that integrate measurable goals and evidence-based interventions, ensuring adaptability to the client’s evolving needs.
• Offer crisis intervention services, including conducting risk assessments, developing safety plans, and effectively coordinating emergency resources for clients in distress.
• Act as a strong advocate for clients' rights, guiding them through healthcare and community resources while ensuring seamless access to services.
• Maintain accurate and confidential client records involving assessments, treatment plans, progress notes, and key outcomes in strict adherence to legal and ethical standards.

This position specifically caters to clients in hospice care who are experiencing terminal illness, their families, and caregivers. • A Master’s degree in Social Work (MSW) from an accredited institution is required.
