• Conduct thorough clinical assessments to understand patients' psychological and emotional needs, followed by the development of comprehensive treatment plans tailored to their unique circumstances.

• Implement solution-focused therapeutic interventions that empower patients and families, utilizing evidence-based practices to address their mental health challenges.

• Maintain up-to-date and accurate clinical documentation in accordance with organizational policies and state guidelines, allowing for quality assurance and compliance.

• Work collaboratively with a team of healthcare professionals, including physicians, nurses, and other specialists, to create an integrated treatment approach that enhances patient care.

• Provide education and support to patients and their families, helping them navigate the complexities of their situations and facilitating connections to community resources.

In this role, you will primarily work with patients receiving hospice care, along with their families, who are facing end-of-life issues. Your expertise in providing emotional support and counseling will be critical in helping them cope with grief, loss, and existential concerns. The ability to connect with individuals from diverse backgrounds and varying life experiences is essential to foster trust and facilitate healing during this sensitive stage of life.

• A Master’s degree in Social Work from an accredited institution, enabling a deep understanding of clinical practices and therapeutic techniques.

• A valid Licensed Specialist Clinical Social Worker (LSCSW) license in the state of Kansas, affirming your capability to practice independently.

• Proficiency in solution-focused psychotherapy and other therapeutic interventions that promote mental health well-being.

• Strong verbal and written communication skills, essential for effective collaboration and documentation.
