Job description
- Provide case consultation and collaborate with interdisciplinary care teams.
- Conduct case management, including assessing patient needs, coordinating care, and connecting patients with community resources.
- Lead group sessions focused on psychoeducation, stress management, mindfulness, and meditation.
- Conduct one-on-one patient visits to provide emotional support, stress management techniques, psychoeducation, and coping strategies.
- Document patient encounters and coordinating follow-up care to support positive health outcomes.
- LCSW license with licensure to practice in GA, FL, and MI (also open to PA, NY, OH)
- 2+ years experience as a an LCSW
- Bachelor's and Master's degrees required
- Experience communicating with patients asynchronously preferred
- Experience with GI conditions or weight management a plus
- Proficiency in using telehealth platforms and electronic health record (EHR) systems
- Passion for working with clients to facilitate behavior change by helping them set health goals to effectively manage chronic disease
- Experience working with patients in a clinic setting preferred
- Demonstrates a willingness to go above and beyond normal responsibilities to provide the best care possible
- Excellent interpersonal communication and problem-solving skills
