• Conduct thorough assessments of patients' mental health status, focusing on emotional, behavioral, and social functioning to design personalized treatment plans.
• Provide psychotherapy tailored to individual needs addressing issues such as anxiety, depression, PTSD, and other mental health conditions.
• Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to coordinate patient care and ensure comprehensive service delivery.
• Monitor and adjust treatment plans based on patient progress and feedback, utilizing data and evidence-based practices to guide clinical decisions.
• Maintain accurate and timely documentation of patient interactions, treatment plans, and assessments in electronic health records (EHR) systems to ensure compliance and facilitate communication.

The target population for this role includes patients within our hospice care program who may experience mental health crises related to terminal illness. Understanding the unique emotional challenges faced by these individuals, the LCSW will play an instrumental role in offering support, enhancing coping mechanisms, and improving overall mental health outcomes during this sensitive time.

• Master’s degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• Exceptional communication and interpersonal skills, fostering strong relationships with patients and collaborative engagements with healthcare teams.
• Proven clinical skills, emphasizing the ability to conduct comprehensive assessments and formulate effective, evidence-based treatment plans.
• Proficiency in using electronic health records (EHRs) to manage patient information and improve service coordination.

• Previous experience working in hospice or palliative care settings is highly desirable.
• Familiarity with trauma-informed care principles and interventions.
• Experience utilizing culturally competent practices within diverse patient populations.
