This role will primarily function in a remote setting, allowing for flexible schedules that accommodate patients’ needs. • Collaborate with primary care providers to identify, address, and manage patients' mental health and psychosocial issues as an integral member of the healthcare team.
• Conduct thorough patient assessments that include both diagnostic and functional evaluations, providing actionable recommendations for treatment goals.
• Deliver targeted interventions for patients seeking mental health services and track their progress towards recovery.
• Guide primary care providers in determining the eligibility of patients for specialized care versus general mental health treatment based on clinical assessment.
• Initiate follow-up procedures to gauge the effectiveness of implemented treatments and adjust plans as needed.
• Develop relapse prevention strategies when warranted and assist patients in navigating their mental health challenges.
• Identify at-risk patients and create effective intervention strategies to improve their overall health outcomes.
• Coordinate with community resources to facilitate comprehensive behavioral healthcare services.
• Document patient progress in treatment charts, maintaining thorough records to ensure continuity of care.
• Maintain an open line of communication with primary care providers, enhancing collaborative treatment efforts.
• Empower patients through self-management educational materials to actively participate in their recovery journey.
• Advise clinic management and team members on best practices in behavioral health services.
• Support adherence to medical treatments prescribed by primary care providers, including managing medication side effects.
• Uphold the Code of Ethical Conduct for Clinical Professionals diligently while performing all duties.
• Embrace additional responsibilities as assigned to enhance clinic operations.

The Licensed Professional Counselor will serve a diverse clientele primarily composed of veterans, their families, and the local community in Round Rock, Texas. Patients may present with a range of challenges, such as PTSD, anxiety, depression, and other behavioral health conditions. The Counselor's role is pivotal in not only addressing immediate concerns but also in fostering long-term emotional resilience and wellness.

• A minimum of a Master’s Degree in Clinical Counseling is required.
• Must hold a current LPC (Licensed Professional Counselor) License.
• Strong knowledge of mental health assessments, treatment planning, and various therapeutic approaches such as family therapy, child psychotherapy, and behavioral therapy.
• Preferred bilingual proficiency in English and Spanish to better serve our diverse patient population.
• Demonstrated mental and physical health suitable for fulfilling the demands of a counseling position.
• In-depth understanding of the personal and social effects of mental health issues on individuals and their families.

• Experience in working within an outpatient mental health clinic or healthcare setting.
• Familiarity with behavioral health integration within primary care practices.
• Previous engagement in community resource coordination is a plus.
