• Conduct thorough bio-psychosocial evaluations to identify and address the mental and emotional barriers facing our patients, ensuring their treatment pathways are effective and accessible.
• Develop individualized treatment plans based on each patient's needs and preferences, employing evidence-based therapies, including both individual and group modalities.
• Provide compassionate outpatient mental health therapy, assisting patients and their families in resolving personal issues stemming from their conditions.
• Document all social work interactions in patients' medical records accurately, adhering to departmental standards and compliance regulations.
• Consult with medical teams regarding patients' psychosocial functioning, working collaboratively to create comprehensive care programs that promote treatment adherence and improve health outcomes.
• Educate patients and their families on social work and medical recommendations, facilitating understanding and compliance in pursuit of optimal functioning.
• Coordinate advocacy needs, leveraging community resources to foster self-determination and support systems outside of the clinic environment.
• Complete departmental reports and statistical documentation to track and demonstrate the efficacy of social work interventions.
• Engage in ongoing professional development to remain current with best practices and methodologies in social work.
• Uphold ethical standards set forth by the NASW Code of Ethics in all patient interactions and clinical decision-making.

In this role, you will primarily work with adult patients who struggle with substance use disorders and related mental health conditions. Your therapeutic strategies will be tailored to accommodate the diverse backgrounds and experiences of our patient population, acknowledging the socio-economic variances that impact their recovery journey.

• A Master's Degree in Social Work from an accredited institution is required, along with a valid license from the State Board of Social Work Licensure at the LCSW level.
• At least one year of clinical experience in a health or mental health setting, with a strong emphasis on working with diverse populations.
• Demonstrated understanding of aging processes and cultural patterns is crucial for providing relevant care across varying life stages.

• Experience working in detox or rehabilitation services, particularly in remote care settings, is highly valued.
• Familiarity with teletherapy platforms and electronic health record systems, enhancing the delivery of care in a virtual context.
