Job description

US Heart and Vascular is in need of a full time Monitoring Care Coordinator - Medical Assistant to join our team at our Value Based Care Call Center in Irving, TX.

Position Summary:

As a Monitoring Care Coordinator (Certified Medical Assistant), this role plays a vital part in our Remote Patient Monitoring (RPM) program assisting cardiology patients to manage their chronic conditions. Responsibilities include monitoring patients' health remotely using digital technology and wearable devices. Oversee patient consent and enrollment in RPM program, deliver patient education, care coordination, manage alerts and collaborate with medical teams in a high-volume call center setting. ย ย 

ย Responsibilities:

  • Welcomes patients to the RPM program.
  • Obtain informed consent and enroll eligible patients in remote monitoring programs. ย 
  • Coordinate the setup and distribution of remote monitoring devices to patients.
  • Educate patients on how to use monitoring devices. ย 
  • Set up monitoring devices in the remote monitoring system via EHR. ย 
  • Review patient readings and alerts regularly and timely, identifying abnormal trends or concerning changes in vital signs.
  • Serve as a primary point of contact for patients enrolled in remote monitoring programs, providing ongoing support, guidance, and encouragement. Communicate regularly with patients to reinforce adherence to monitoring protocols. ย 
  • Effectively communicate and collaborate with providers and patient care teams to ensure seamless care delivery. ย 
  • Monitor vital sign alerts during business hours and screening patients need to ensure a timely response to any changes in health care status or needs ย 
  • Coordinating patient care and collaborating with other members of the healthcare team through electronic medical records. ย 
  • Maintain accurate and up-to-date records of patient interactions, monitoring activities, make collaborative efforts with medical teams and outcomes using electronic health record (EHR) systems.ย 
  • Performs other job-related duties as assigned.ย 

Requirements:

  • Certified Medical Assistant (CMA) Certification
  • Previous experience in a healthcare setting, preferably with monitoring services ย 
  • Demonstrate strong technical skills with monitoring devices ย 
  • Understanding of medical terminologyย 
  • Excellent organizational skills and attention to detail. ย ย 
  • Strong communication and interpersonal skills. ย 
  • Ability to work independently and collaborate effectively with a team.
