This remote role offers the flexibility to work from your home while collaborating with a dynamic multidisciplinary team. Your primary setting will be within a community health clinic framework, focusing on senior populations. While most interactions will be virtual, occasional meetings or training sessions may occur, encouraging connection among team members.

As a Licensed Clinical Social Worker, your daily responsibilities will include:

• Conducting comprehensive evaluations of seniors’ psychosocial needs, considering their emotional, mental, and social well-being.
• Creating tailored care plans in collaboration with clients and their families that align with their individual goals and aspirations.
• Providing counseling sessions to seniors, offering emotional support during life transitions, such as illness, loss, or relocation.
• Advocating for clients by connecting them with community resources and navigating healthcare systems to ensure they receive necessary services.
• Coordinating care with an interdisciplinary team, including healthcare providers and caregivers, to optimize client outcomes and streamline communication.
• Scheduling and conducting regular follow-ups to monitor clients’ conditions, living situations, and addresses any emerging concerns.
• Educating families on available support networks and facilitating access to social engagement activities to enhance their overall quality of life.
• Maintaining accurate and up-to-date records of client interactions, assessments, and progress notes to ensure compliance with regulatory standards.

This role primarily focuses on seniors and their families, who often face unique challenges related to aging. Your work will provide critical support to this demographic, equipping them with necessary tools and resources to navigate health transitions and maintain a meaningful quality of life. • A Master’s degree in Social Work from a regionally accredited institution is required.
• A current and valid state license as a Clinical Social Worker (LCSW).
• At least two years of relevant experience in a social work capacity, ideally within a community health or geriatric context.
• Proficient understanding of social work principles, community resources, and issues related to aging.
• Demonstrated ability to empathize and build rapport with clients and families, maintaining sensitivity to their needs.
• Strong communication, assessment, and counseling skills.
• Experience with electronic health record (EHR) systems, facilitating efficient client interaction documentation.

While a minimum of two years of experience is required, candidates with a background in community health settings or work focused on the geriatric population will have a competitive advantage.
