Work Environment

This remote position is developed in a collaborative setting, allowing for flexibility and adaptability in your work schedule, which may include day, evening, and weekend shifts depending on departmental needs. The role entails participation in on-call responsibilities, ensuring patient support is available 24/7. Key Responsibilities

• Conduct comprehensive psychosocial assessments to identify and articulate patients' emotional and social needs.
• Provide individualized and group counseling, focusing on issues such as adjustment, grief, anxiety, and coping strategies.
• Collaborate with medical teams to develop holistic care plans that incorporate the psychological aspects of treatment.
• Connect patients with appropriate community resources, facilitating post-discharge services to ensure continuity of care.
• Offer educational resources and emotional support to patients and their families, empowering them to make informed decisions about treatment.
• Maintain accurate and timely documentation in accordance with legal and hospital policies to support continuity of care.
• Participate actively in hospital committees to enhance social work practices and influence patient-centered care initiatives.

Patient Population / Client Focus

This role primarily focuses on patients experiencing anxiety and trauma-related disorders, often within the context of a detoxification process. You will engage with individuals confronting significant life changes, emotional distress, and various psychosocial challenges. Your ability to empathize and provide targeted support will be essential in promoting positive patient experiences and outcomes.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong foundation in clinical social work principles and practices, particularly in a healthcare setting.

Preferred Experience

• A minimum of two years of experience working in a healthcare or hospital setting, with a preference for experience in mental health or addiction services.

Compensation and Benefits
