Job description

PCN Care Coordinator

PCN Care Coordinators play an important role providing extra time, capacity and expertise to support patients to be actively involved in managing their care and supported to make the choices that are right for them. Working closely with GPs, GP practice teams and other health and care professionals and organisations involved in care, they act as a central point of contact for patients.

Please note that the role of a care coordinator is not a clinical role.

Main Duties Of The Job

The following are the core responsibilities of the PCN Care Coordinator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

  • To identify cohorts of patients and signpost to the relevant services.
  • To support the coordination and administration of all Meridian clinics i.e. Weight Management, Menopause & Pain Management sessions.
  • Adhoc administration tasks and clinic booking for GP practices.
  • Ensuring patients records are updated and correctly coded.
  • Support the coordination and administration of MDT meetings, taking and distributing minutes, bringing information and feedback to others where needed within Meridian PCN.
  • Administrative support for all PCN Projects.
  • To reduce administrative burden on GPs and other clinicians.
  • To attend the MAF (Multi Agency Forum) meetings when needed and have information ready for this
  • Check coding is correct in SystmOne & Emis
  • Begin the Patient Passport with Proactive Neighbourhood Patients

Job Responsibilities

Although this is predominantly a remote working role, you will be expected to attend in person, mandatory monthly team meetings, PCN Meetings, our practices on occasion and as part of your role travel within our community to patients homes. You may also be asked to support at a group session at short notice. Due to the locations of our surgeries it is essential that you have access to your own transport.

It is also essential that candidates applying have primary care experience, community experience is also desirable.

This role is intended to become an integral part of the PCNs multidisciplinary team, working alongside social prescribing link workers and health and wellbeing coaches to promote and embed a personalised care approach.

You must be able to work independently as well as part of a team, be a forward thinking, caring & dedicated A reliable and person-focused individual who enjoys working with a wide range of people. You will have a strong working knowledge of Office 365 programs and general IT, good written and verbal communication skills and proven strong organisational and time management skills.As this is an evolving role, we are looking for highly motivated and proactive applicants with a flexible can-do attitude, keen to work and learn as part of a team, and who will be committed to providing patients and practices with high quality support.
