We are seeking compassionate, detail-oriented Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) to join our Remote Patient Monitoring (RPM) team. This is a fully remote position where you will monitor patients' vital signs, identify clinical trends, and collaborate with providers to improve patient outcomes.
Responsibilities:
Monitor daily patient vital signs through our Remote Patient Monitoring platform.
Review blood pressure, pulse, oxygen saturation, weight, blood glucose, temperature, and other biometric data.
Identify abnormal trends and escalate clinically significant findings to the appropriate provider.
Communicate with patients by phone to assess symptoms, provide education, and encourage adherence to care plans.
Document all patient interactions and clinical interventions accurately in the electronic medical record.
Collaborate with nurse practitioners, physicians, and the clinical team to ensure timely follow-up.
Meet Medicare RPM documentation requirements while delivering high-quality patient care.
Qualifications:
Active RN or LPN license in good standing.
Strong assessment and critical thinking skills.
