You will have access to a comprehensive suite of digital tools that facilitate telehealth services, client documentation, and communication with families and healthcare teams. • Conduct comprehensive assessments to identify the psychosocial needs of seniors, taking into account emotional, mental, and social factors that affect their well-being.
• Develop and implement personalized care plans in collaboration with clients and their families, ensuring that these plans align with their individual goals and preferences.
• Provide counseling and emotional support, assisting seniors and their families in navigating transitions, such as health challenges or changes in living conditions.
• Advocate for clients' rights and accessibility to necessary community resources, linking them with services essential to their well-being.
• Work closely with an interdisciplinary team, including health professionals, caregivers, and educators, to create a holistic support system for each client.
• Conduct regular virtual visits to monitor clients’ progress, address any emerging concerns, and ensure a safe and supportive environment.
• Educate clients and their families about available resources, support groups, and community programs that can enhance their engagement and quality of life.
• Maintain up-to-date and accurate records of assessments, care plans, and client interactions, ensuring compliance with ethical standards and organizational policies.
• Participate in staff meetings and ongoing training sessions to stay informed about best practices in social work and aging.

Your primary focus will be on seniors, many of whom may face unique challenges related to aging, including mental health concerns, social isolation, and difficulties in accessing healthcare services. You will play a vital role in addressing these issues through tailored interventions and fostering strong relationships with clients and their families.

• A Master’s degree in Social Work from an accredited institution is mandatory.
• Current and valid LMSW license to practice as a social worker in the state of Florida.
• A minimum of two years of relevant social work experience, ideally in a community center or geriatric settings.
• Strong understanding of aging-related issues, social work principles, and available community resources.
• Exceptional verbal and written communication skills, alongside a knack for empathetic listening and rapport building.
• Proficiency in electronic health record (EHR) systems and Microsoft Office suite to ensure smooth documentation and communication.

• Experience working remotely, utilizing digital communication platforms effectively.
• Familiarity with telehealth practices, particularly in providing remote support to senior clients.
• Training in conflict resolution and mediation skills.
