• Conduct comprehensive clinical assessments to evaluate the needs of patients and families, formulating individualized treatment plans that align with best practices and solution-focused approaches.
• Utilize evidence-based therapeutic interventions to address mental health concerns, including grief, anxiety, and depression, fostering a safe space for patients to express their feelings.
• Maintain meticulous clinical documentation, ensuring compliance with legal and ethical standards, while also allowing for effective continuity of care.
• Collaborate closely with a multidisciplinary team, including physicians, nurses, and other social workers, to provide integrated and coordinated patient care.
• Participate in regular case consultation sessions and peer review meetings aimed at enhancing service delivery and improving patient outcomes.
• Educate patients and their families about coping mechanisms and available support resources, empowering them with knowledge to navigate their circumstances.
• Stay informed on the latest research and developments in psycho-social care, ensuring that the interventions and services provided are relevant and effective.

You will primarily work with terminally ill patients and their families in a hospice setting. This role requires sensitivity and a strong understanding of the emotional complexities surrounding end-of-life care. You will be a crucial support system for patients facing significant life challenges, focusing on providing comfort, dignity, and peace during their remaining time.

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license to practice in Georgia.
• A minimum of three years of clinical experience in a mental health setting, with specific experience in end-of-life care preferred.
• Demonstrated proficiency in solution-based psychotherapy interventions tailored to diverse patient needs.
• Strong communication, interpersonal, and problem-solving skills, with the ability to work collaboratively in a multidisciplinary team.

• Experience working with hospice or palliative care populations is highly desirable.
• Familiarity with electronic health record (EHR) systems to ensure effective patient documentation and follow-up.
• Previous involvement in community outreach or education efforts surrounding mental health and end-of-life planning.
