Key Responsibilities
  • Conduct thorough and ongoing assessments to determine the specific needs of patients, ensuring personalized and effective treatment plans.
  • Facilitate regular communication among supervisors, direct care staff, family members, guardians, and relevant stakeholders to provide updates on individual progress and adjustments to therapy as needed.
  • Develop a tentative schedule that outlines upcoming appointments, home visits, and observational assessments to monitor and enhance client engagement.
  • Coordinate essential home visits for in-home assessments, which might include chart reviews, interviews with staff members, and necessary data analyses to enrich treatment outcomes.
  • Ensure effective implementation of individualized program plans while adhering to best practices in mental health care.
  • Actively participate in Interdisciplinary Team Meetings and uphold the collaborative standards established in our clinic.
  • Complete monthly and quarterly progress notes, documenting patient advancements, and modifications to treatment plans as necessary.
  • Organize and lead training sessions to ensure all staff members are proficient in the implementation of program plans and understand documentation requirements.
  • Provide consistent follow-up interviews and home visits to offer support and address any evolving client needs.
  • Maintain accurate and accessible records, ensuring confidentiality and compliance with legal standards.
  • Serve as a liaison with the public and the broader mental health community to advocate for continued development and support of mental health initiatives.
  • Assist in the development of training materials and procedures to continuously improve program offerings for clients.
