Work Environment

This position is entirely remote, allowing for flexibility in your work schedule while maintaining a strong connection with clients and team members. Key Responsibilities
  • Conduct initial assessments to discuss clientsโ€™ goals, preferences, and available options prior to formulating individualized rehabilitation plans.
  • Develop and implement rehabilitation plans tailored to each client's education level, aptitudes, physical capabilities, and career objectives, modifying them as necessary during the course of treatment.
  • Collaborate with an interdisciplinary team of healthcare professionals, including physicians, psychologists, and occupational therapists, to ensure the creation and execution of comprehensive treatment programs that meet the unique needs of each client.
  • Maintain accurate and confidential case files, documenting essential client information such as eligibility, insurance details, services rendered, and therapy sessions conducted.
  • Monitor client progress closely, documenting any changes and assessing whether treatment goals are being achieved, adjusting plans as needed to ensure continued success.
  • Engage consistently with clients participating in job training and occupational programs, providing proactive resolution for any challenges they face during their placements.
  • Work collaboratively with clientsโ€™ families to identify support systems and resources that can further assist client needs throughout their rehabilitation journey.
  • Prepare timely and insightful analysis reports on client needs and progress, relaying information crucial for ongoing treatment efforts.
  • Make referrals to external resources as needed, ensuring clients have access to comprehensive support services.
Patient Population / Client Focus

The LPC will work primarily with adults and adolescents facing a variety of mental health challenges, including mood disorders, anxiety disorders, and other psychological conditions. Qualifications
