As a Licensed Clinical Social Worker at Beyond the Edge, you will have varied responsibilities critical to patient success, including:

• Conducting thorough psychosocial assessments through detailed interviews, direct observations, and comprehensive medical record reviews to identify and evaluate emotional, social, and environmental factors affecting patient recovery.
• Offering individual and family counseling to address adjustment challenges and emotional well-being, thereby facilitating coping mechanisms and resilience in patients dealing with their rehabilitation journey.
• Collaborating closely with the rehabilitation team, including occupational and physical therapists, to create individualized treatment plans that incorporate each patient's psychosocial needs and promote coordinated care.
• Guiding patients in accessing community resources such as vocational rehabilitation services, support groups, and housing assistance, ensuring that all aspects of their recovery are addressed.
• Educating and supporting families about navigating post-rehabilitation services and community resources to ensure a smooth transition from care.
• Playing a vital role in discharge planning, focusing on facilitating seamless transitions for patients back to their homes or other care environments.

The patients you will serve encompass a broad spectrum, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Your empathic approach and specialized skills will empower patients to confront their challenges and foster their recovery journey toward achieving the highest level of independence.

• A Master's degree in Social Work from an accredited institution.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.
• Demonstrated understanding of psychosocial dynamics, including adjustment issues, grief, and coping mechanisms.
• Exceptional communication and interpersonal skills, enabling effective interaction with patients and multidisciplinary teams.

• Experience working in a rehabilitation or medical setting is highly desirable, as this role requires familiarity with such environments and patient conditions.
• Ability to apply understanding of community resources and post-rehabilitation services effectively.
