Job Summary:

The LPN Care Manager supports patients through telephonic outreach, care coordination, and ongoing health education. Responsibilities include monitoring care plans, scheduling appointments, reviewing medical records, documenting in EHRs, and facilitating access to medical services. Strong communication, organization, and computer skills are essential.


Essential Functions: (may include, but not limited to):

  • Manage and support patients' healthcare needs via telephonic outreach
  • Conduct and document visits by using several technology platforms and EHRs
  • Monitors adherence to care plans, evaluates effectiveness, monitors patient progress in a timely manner, and facilitates changes as needed.
  • Facilitates patient access to appropriate medical and specialty providers.
  • Educates patients and family/caregiver(s) about relevant community resources.
  • Coordinates continuity of patient care with external healthcare organizations and facilities including from the primary care provider to a specialty care provider.
  • Supports patient self-management of disease and behavior modification interventions.
  • Provides patient health counseling, education and instruction.
  • Any other duties necessary to drive our values, fulfill our mission, and abide by our company policies.
  • Using computer applications
  • Making and answering phone calls the majority of the shift
  • Welcoming patients
  • Reviewing patient medical records
  • Coding and filling out insurance forms

Required Skills/Abilities:

  • New York LPN licensure is required. Compact LPN (Desired)
    • Maintaining a License is an ongoing requirement of this role
  • Minimum of 2-years work experience in a healthcare setting involving patients with complex chronic disease states preferred.
  • Cronic Care Managment experience preffered.
  • Proficiency with a personal computer
  • Technical and analytical skill set
  • Excellent communication, organizational, and interpersonal skills are necessary
  • Experience with Microsoft Excel preferred

Total Rewards:

The target salary range for this role is $20 - 26per hour. An individual's salary within this range is based on multiple factors including but not limited to skills, experiences, licensure, certifications, and other business and organizational considerations.
