This is an advanced level Patient Aligned Care Team (PACT) Licensed Practical Nurse (LPN) position in the primary care service located at the various associated community based outpatient clinics within the Birmingham VA healthcare system.

Duties

The Licensed Practical Nurse (LPN) performs assigned duties in a manner that promotes quality health care service, positive customer service, and the maintenance of an environment committed to serving the Veterans.
Assignment duties may include but not limited to:

• Manages patient care problems for a wide range of patients including those with chronic and acute illness.
• Assesses, records and reports vital signs neurological signs weight and blood glucose levels.
• Checks patients into the clinic and documents intake data.
• Provides high quality nursing care using traditional and non-traditional modalities.
• Demonstrates the ability to independently adjust or modify care being delivered.
• Serves as a resource for complex slash specialized procedures.
• Demonstrates knowledge of standard precautions and sterile techniques.
• Demonstrates the knowledge and skill to interpret common test results and other medical data.
• Performs clinical pertinence reviews to ensure compliance with nursing practice standards.

Administrative duties may include but not limited to:

• Completes all required documentation in a timely manner and in accordance with VA policy and TJC standards of care.
• Remain current on all changes, improvements, and software patches related to VISTA and CPR systems.
• Actively participates in continuous and ongoing performance improvement projects and/or primary care committees.
• Assists Nurse Manager (NM) with management of encounters
• Maintains current annual and professional licensure certifications, which includes all state licensure requirements for LPN's and BLS certification.
• Leads PACT huddles effectively and serves as a resource for new clinical and administrative staff members.

Medication Administration duties may include but not limited to:

• Prepares, administers, and documents medications and immunizations as directed without error.
• Observes for subtle physical, emotional side effects, or contraindications related to medications when interviewing patients.
• Assists with medical reconciliation to improve patient safety.
• Collaborates with RN, provider, and/or pharmacist to address patient medication questions and/or changes in patient conditions from prescribed medications/treatments.

Work Schedule: Monday - Friday, 7:30am - 4:00pm. Work schedule may change based on agency needs.
Telework: Ad-hoc, as authorized by supervisor.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:

• United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
• Individual Occupational Requirement (IOR) Candidates must be currently licensed to practice as practical or vocational nurses in a state or territory of the United states or the District of Columbia or must have applied for a license to practice. Applications of candidates possessing a license must be accompanied by a certified or photostatic copy of the license, a notarized statement attesting to the fact, or a citation of the license number and state issuing it. Non-licensed candidates who have completed their training may be appointed subject to obtaining the required license during the probationary period. Those not licensed must furnish the date on which they apply for a license and the jurisdiction involved. No person appointed pending licensure may be retained beyond the probationary period if licensure has not been attained.

Experience

• For GS-5 and above: 1 year of nursing experience equivalent to at least the next lower grade level is required.

The full performance level of this vacancy is GS-6. The actual grade at which an applicant may be selected for this vacancy is GS-5.

Receiving Service Credit for Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee.
