• Conduct comprehensive mental health assessments that incorporate the impact of physical health conditions on emotional well-being.
• Design and implement integrated treatment plans that address both psychiatric and medical needs based on clinical guidelines and evidence-based practices.
• Facilitate individual and group therapy sessions utilizing a variety of therapeutic modalities tailored to each patient's circumstances, including cognitive behavioral therapy (CBT) and dialectical behavior therapy (DBT).
• Collaborate effectively with medical providers to ensure seamless care coordination and optimize treatment outcomes for patients.
• Provide crisis intervention services to support patients experiencing mental health emergencies, ensuring timely and appropriate care.
• Educate patients about the interrelation between mental and physical health, equipping them with strategies to manage their conditions holistically.
• Maintain accurate, confidential documentation in compliance with HIPAA regulations and professional ethical standards.
• Engage in continuous professional development by staying informed about current research and best practices in integrated care.

Your work will center around a diverse patient population dealing with complex medical and mental health issues, which may include chronic illnesses, mental health disorders, and substance use issues. • A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution is required.

• Strong proficiency in addressing the mental health needs of individuals with co-occurring medical conditions.
• Proven experience delivering integrated care and collaborating with medical professionals to improve patient outcomes.
• Exceptional interpersonal and communication skills, with a demonstrated ability to connect with diverse clientele.
• Expertise in maintaining professional boundaries, ensuring ethical compliance, and upholding patient confidentiality.
• Familiarity with electronic health record (EHR) systems and telehealth platforms to facilitate patient care effectively.
• A strong commitment to working collaboratively as part of an interdisciplinary team.

• At least 2 years of relevant experience as a licensed mental health therapist in a clinical or outpatient setting.
• Experience working in a telehealth environment is highly desirable.
• Training in trauma-informed care practices and methodologies.
