• Conduct thorough assessments for each client, identifying their unique mental health needs and developing tailored treatment plans.
• Engage in continuous communication with supervisors, family members, and other stakeholders regarding the client’s progress and treatment adjustments.
• Create and manage a tentative schedule for appointments, home visits, and other necessary follow-ups to ensure comprehensive care.
• Coordinate home visits to provide in-home observations, perform chart reviews, interview staff, and analyze data related to client progress.
• Implement and monitor the effectiveness of program plans, ensuring all interventions are based on best practices and are culturally competent.
• Participate in Interdisciplinary Team Meetings, contributing insights and strategies for holistic client support.
• Document progress notes monthly and quarterly, ensuring that all records reflect accurate and timely information.
• Organize training sessions for staff and team members, focusing on competencies necessary for executing program plans and maintaining documentation standards.
• Provide ongoing support through follow-up interviews and continued observations to adapt treatment plans based on client feedback and progression.
• Act as a liaison within the mental health community, advocating for client needs and program development goals.
• Help in creating and improving procedures for training materials that enhance our service delivery.

In this role, you will work with a diverse client population, including children, adolescents, and adults facing a variety of challenges, such as family issues, trauma, substance misuse, and mental health disorders. Your expertise will empower clients from various backgrounds to navigate their experiences and strive for better mental health outcomes.

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
• Active and unrestricted Professional Counselor (LPC) licensure in Tennessee.
• A minimum of 2 years of counseling experience, preferably in clinical settings, demonstrating a strong commitment to mental health care.
• Expertise in areas such as family therapy, couples therapy, substance misuse, or trauma is highly desirable.
• Robust knowledge of legal and ethical standards pertaining to counseling services.
• Exceptional communication and interpersonal skills, enabling you to build rapport with a wide variety of clients.
• Proven ability to collaborate effectively with a healthcare team to promote integrated care.
• Familiarity with electronic medical records (EMR) software and related technologies.

• Experience working within a community health setting is preferred, as it relates to understanding diverse client needs.
• Training or certifications in trauma-informed care, cognitive behavioral therapy (CBT), or similar methodologies are beneficial.
