Vision Works Youth Counseling Center is seeking a passionate Licensed Independent Clinical Social Worker (LICSW) to join our dedicated team on a full-time, remote basis. This position is set in a fully remote work environment, allowing for flexibility while supporting the operational needs of Vision Works. Our work setting includes a detox center, where our social workers collaborate with a multidisciplinary team to provide holistic care aimed at recovery and wellness. As part of a remote team, you will utilize virtual communication tools to maintain connections with clients and colleagues, ensuring continuous care and support.

• Conduct detailed assessments to evaluate the psychosocial needs of clients, particularly seniors, considering their emotional, mental, and social circumstances.
• Develop individualized care plans in collaboration with clients and their families, tailored to address their specific goals, challenges, and preferences.
• Provide counseling sessions focused on emotional support for seniors and their families, facilitating discussions around life transitions and essential coping strategies.
• Advocate for clients' rights and assist them in accessing vital community resources to ensure they receive the needed support and services.
• Collaborate with an interdisciplinary team of healthcare professionals, including caregivers and medical staff, to optimize care coordination and improve client outcomes.
• Conduct regular check-ins and home visits to monitor clients' living conditions and well-being, addressing any emerging concerns or safety issues promptly.
• Facilitate family communications to resolve conflicts that may arise during the care process, promoting unity and understanding in support of shared care goals.
• Educate clients and their families about available community support services, programs, and engagement opportunities to enhance their quality of life.
• Maintain precise and up-to-date documentation regarding assessments, care plans, interactions, and client progress in accordance with regulatory standards.
• Stay informed about current best practices, policies, and regulations in social work to ensure compliance and uphold ethical standards in all interactions.

The primary focus of this role is on seniors and their families, catering to individuals facing challenges related to aging, mental health, and social isolation. The complexity of issues often encountered includes anxiety, depression, and the need for transition support as clients navigate changing life circumstances. Your work will significantly aid clients in regaining control over their lives, ultimately leading to enhanced emotional well-being and quality of life.

• A Master’s degree in Social Work from an accredited institution.
• A current and valid state license as a Licensed Independent Clinical Social Worker (LICSW).
• At least two years of proven experience in a social work setting, ideally within community health or geriatric services.
• Strong knowledge of social work principles, challenges pertaining to aging, and familiarity with local community resources.
• Exceptional assessment, counseling, and problem-solving abilities.
• Empathy, compassion, and the capability to forge strong relationships with senior clients and their families.
• Outstanding communication and interpersonal skills for effective collaboration with fellow team members.
• Proficient in electronic health record (EHR) systems and relevant computer software.

• Experience working in multidisciplinary teams and knowledge of interdisciplinary approaches in social work.
• Familiarity with telehealth systems and virtual counseling methods.
• Background in community program development and implementation, especially related to aging population services.
