• Conduct thorough psychosocial assessments to determine patients’ emotional, social, and financial needs, facilitating tailored therapy interventions.
• Deliver individual and group counseling focused on various issues including adjustment disorders, grief, and coping mechanisms, thereby fostering resilience in our patients.
• Collaborate with interdisciplinary medical teams to formulate and execute comprehensive care plans aimed at improving patient health outcomes.
• Coordinate post-discharge services and connect patients with community organizations to ensure a continuum of care.
• Provide education and emotional support to patients and their families regarding mental health treatment options and available resources.
• Engage in crisis intervention when necessary, offering immediate support to patients facing urgent psychosocial challenges.
• Evaluate and document patient progress, maintaining detailed records that comply with hospital policies and legal standards.
• Contribute social work expertise to hospital committees, enhancing the integration of mental health considerations into broader health initiatives.
