• Conduct thorough clinical assessments to evaluate patients' mental health needs and develop targeted treatment plans that address individual circumstances.
• Implement evidence-based solution-focused interventions, including counseling and therapeutic techniques designed to support patients and their families.
• Maintain accurate and timely documentation of clinical records in accordance with regulatory standards, ensuring compliance and confidentiality.
• Engage in active collaboration with a multidisciplinary care team, including nurses, physicians, and spiritual care providers, to create a cohesive plan of care.
• Provide education and resources to patients and families, helping them navigate the emotional complexities associated with hospice care.
• Stay abreast of the latest research and best practices in mental health and hospice care to continuously enhance treatment approaches.

Our patient population consists primarily of individuals with terminal illnesses as well as their families, who often experience a range of emotional responses, from grief to anxiety. As an LCSW in our hospice center, you will provide critical support to these patients, facilitating conversations about their needs and preferences while guiding their loved ones through complex emotional landscapes.

• A Bachelor's degree in Social Work from an accredited institution is required; a Master's degree is preferred.
• A valid LCSW license in the state of Tennessee is necessary for this role.
• Familiarity with solution-focused therapeutic approaches is essential.
• Strong communication, interpersonal, and problem-solving skills to effectively interact with patients, families, and interdisciplinary team members.

While a minimum of two years of relevant experience is required, candidates with a background in hospice or palliative care settings will be highly preferred. Experience working with diverse populations and knowledge of mental health treatments and interventions are also beneficial.
