You will be part of a multidisciplinary team dedicated to integrated care, where communication and coordination are key components in delivering high-quality mental health services to our patients.

• Conduct comprehensive bio-psychosocial evaluations to identify patient needs and barriers to treatment, using your clinical expertise to enhance overall health status.
• Work closely with our social work manager to assess and treat diverse psychosocial issues, helping patients and their families navigate complex emotional and environmental challenges linked to their health conditions.
• Employ evidence-based treatment modalities as you develop tailored treatment plans and initiate therapeutic interventions, utilizing both individual and group treatment approaches.
• Document all social work activities and patient interactions in accordance with departmental standards, ensuring accurate records for ongoing treatment processes.
• Collaborate with medical and support staff to design and implement comprehensive care programs aimed at improving patient engagement and health outcomes.
• Provide education to patients and their families, equipping them to understand and embrace social work and medical recommendations, thereby promoting optimal recovery and well-being.
• Remain informed of community resources and advocacy channels to assist patients in accessing necessary services to support their social and emotional needs.
• Implement continuous professional development by engaging in training opportunities tailored to your specific clinical responsibilities.

Your role as an LCSW will involve working intimately with patients experiencing a spectrum of psychosocial issues, particularly those related to chronic illness and end-of-life concerns in our hospice environment. You will engage with diverse populations, addressing their specific needs while employing culturally sensitive approaches to therapy.

• A Master’s Degree in Social Work from an accredited institution is required, along with active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Maryland.
• A solid understanding of human development, aging processes, and how various cultural patterns affect mental health care delivery across different demographics.

• At least six months to one year of clinical experience, preferably in a healthcare or mental health setting that serves similar populations to those at Clear Sky Family & Marriage Counseling.
• It is advantageous to have training that addresses the particular needs of patients related to their developmental stages and social circumstances.

This full-time position offers an annual salary in the range of USD 85,000 to 100,000, commensurate with experience and qualifications.
