This position is designed for a remote work setting, allowing flexibility to meet the needs of our clients. Despite the virtual nature of the role, you will be part of a collaborative multidisciplinary team that includes psychiatrists, nurses, and other healthcare professionals. • Conduct thorough psychosocial assessments to evaluate each patient's mental health status, strengths, needs, and personal circumstances, ensuring that care is individualized.
• Provide evidence-based therapies, such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), to effectively address a range of mental health concerns.
• Collaborate with an interdisciplinary team to develop and implement comprehensive treatment plans that align with best practices and reflect the patient’s goals.
• Assist patients and their families in understanding diagnoses, exploring treatment options, and managing medication regimens, fostering a supportive environment for recovery.
• Facilitate individual, group, and family therapy sessions, utilizing evidence-based modalities to promote healing and growth.
• Advocate for patients within the mental health system, assisting them in navigating resources and ensuring that their rights and needs are met effectively.
• Prepare and lead therapeutic group sessions and psychoeducation workshops, focusing on skills development and emotional regulation.
• Enhance discharge planning meetings to ensure continuity of care post-treatment and facilitate smooth transitions for patients returning to community life.

You will be serving a diverse client population within our community health clinic, where patients may present with a wide range of mental health challenges, including anxiety, depression, trauma, and substance abuse issues. Your expertise will be crucial in developing individualized treatment plans that respect and integrate each patient's unique background, culture, and life circumstances.

• A Master’s degree in Social Work from an accredited program is required.
• Must hold active state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrated knowledge of mental health disorders, psychotherapeutic modalities, and crisis intervention strategies.
• Exceptional ability to collaborate effectively within a multidisciplinary team, contributing to the overall care of patients.

• Previous experience in a community health setting or working with underserved populations is highly desirable.
• Familiarity with electronic health record systems and telehealth platforms for efficient patient management.
• Experience in facilitating group therapy or psychoeducation sessions would be a valuable asset.
