Work Environment

This remote position allows you to work from home while being actively involved in a community health clinic setting. Key Responsibilities

  • Collaborate closely with clients and their families to develop tailored care plans that address individual preferences and goals.
  • Provide empathetic counseling and emotional support to seniors, assisting them in navigating life transitions and challenges such as grief, loss, and adjustment to aging.
  • Advocate for clients by facilitating access to essential community resources and support services, ensuring they receive the appropriate care.
  • Work as part of a multidisciplinary team to coordinate comprehensive care, connecting with healthcare professionals to enhance service delivery.
  • Monitor client well-being through regular check-ins, assessing home environments, and addressing any potential risks or concerns.
  • Guide families through difficult conversations and support conflict resolution related to the care of senior clients.
  • Educate clients and their families on available community resources, support networks, and programs aimed at improving social engagement and overall quality of life.
  • Maintain detailed, accurate, and up-to-date documentation of client assessments, care plans, and progress notes in compliance with established compliance standards.
  • Stay informed about current best practices, policies, and regulations relevant to clinical social work to uphold ethical service delivery.

Qualifications
