This remote position allows you to work from home while being actively involved in a community health clinic setting. You will be part of an interdisciplinary team, collaborating with caregivers, nurses, and healthcare professionals to ensure each client receives holistic care tailored to their unique psychosocial health needs. Flexibility is a core aspect of this role, with work hours typically scheduled from Monday to Friday, 8:00 AM to 4:30 PM, and occasional evenings or weekends as client needs arise.

• Conduct in-depth assessments of clients' psychosocial and emotional needs, particularly for senior clients, considering their mental health and social circumstances.
• Collaborate closely with clients and their families to develop tailored care plans that address individual preferences and goals.
• Provide empathetic counseling and emotional support to seniors, assisting them in navigating life transitions and challenges such as grief, loss, and adjustment to aging.
• Advocate for clients by facilitating access to essential community resources and support services, ensuring they receive the appropriate care.
• Work as part of a multidisciplinary team to coordinate comprehensive care, connecting with healthcare professionals to enhance service delivery.
• Monitor client well-being through regular check-ins, assessing home environments, and addressing any potential risks or concerns.
• Guide families through difficult conversations and support conflict resolution related to the care of senior clients.
• Educate clients and their families on available community resources, support networks, and programs aimed at improving social engagement and overall quality of life.
• Maintain detailed, accurate, and up-to-date documentation of client assessments, care plans, and progress notes in compliance with established compliance standards.
• Stay informed about current best practices, policies, and regulations relevant to clinical social work to uphold ethical service delivery.

This role primarily focuses on the senior population, addressing their unique needs in mental health and social integration. A deep understanding of the issues faced by aging individuals is crucial, as is the ability to effectively communicate and provide support to families involved in their care. By empowering seniors, you will contribute meaningfully to their emotional resilience, helping them regain control and improve their quality of life.

• A Master's degree in Social Work from an accredited institution is mandatory.
• Must possess current, valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of relevant experience in social work, preferably within a community health or geriatric setting.
• Thorough knowledge of social work principles, community resources for seniors, and aging-related issues.
• Outstanding assessment, counseling, and problem-solving abilities.
• Demonstrated empathy, compassion, and ability to engage effectively with clients and their families.
• Strong communication skills, both verbal and written, for effective collaboration with diverse teams and stakeholders.
• Proficiency with electronic health record (EHR) systems and relevant computer applications to maintain comprehensive client documentation.

Experience in a community health clinic or geriatric social work setting is preferred, as this demonstrates familiarity with the specific challenges and circumstances faced by seniors.
