Key Responsibilities
  • Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, thereby functioning as an integral part of the patient care team.
  • Conduct comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations, and recommend treatment goals and plans to primary care providers.
  • Deliver brief, focused mental health interventions for patients in need, tailoring approaches to individual treatment plans.
  • Guide primary care providers on referrals to specialized mental health facilities when patient needs exceed standard clinic capabilities.
  • Initiate follow-up communications to evaluate patient progress and effectiveness of treatment strategies, adapting approaches as necessary.
  • Develop and implement relapse prevention plans to support patients in maintaining stable functioning, focusing on sustainable mental health practices.
  • Monitor patient health services delivery related to behavioral health care, coordinating with external treatment providers with patient consent.
  • Document patient progress thoroughly in treatment charts, ensuring compliance with ethical and legal standards.
  • Keep primary care teams informed about patient needs and treatment progression, fostering collaborative treatment planning.
  • Empower patients by equipping them with self-management skills and educational resources, fostering independence in their recovery journey.
