Your role will involve regular communication with healthcare professionals and family members through telehealth platforms and virtual meetings. Additionally, you will have the opportunity to visit different facilities as necessary to ensure holistic support for patients enrolled in hospice care.

• Conduct comprehensive mental health assessments for patients, considering their medical history, psychosocial factors, and presenting concerns.
• Develop and implement individualized treatment plans that address mental health issues while promoting emotional well-being and coping strategies for patients and their families.
• Provide evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and supportive counseling, tailored to meet the unique needs of each patient.
• Facilitate individual and group therapy sessions, focusing on bereavement support, emotional regulation, and effective coping mechanisms for patients and their loved ones.
• Collaborate with interdisciplinary teams, including physicians and nurse practitioners, to ensure coordinated and comprehensive care that aligns with each patient's goals and wishes.
• Document patient progress and collaborate on referrals to additional services as needed, ensuring continuity of care.
• Engage in community outreach to raise awareness of hospice services and available mental health resources.
• Provide crisis intervention as necessary, and be on call during designated periods to respond to urgent situations that may arise.

In your role, you will work directly with patients facing terminal illnesses and their families. The focus will be on addressing emotional and psychological challenges associated with end-of-life care, including anxiety, depression, grief, and the unique dynamics of family interactions during this critical time. Your expertise in mental health will be invaluable in supporting patients to find meaning and peace in their experiences, while also aiding family members in navigating their own emotional responses.

• A Master's degree in Social Work (MSW) and a current Licensed Clinical Social Worker (LCSW) credential.
• Strong understanding of hospice and palliative care practices.
• Exceptional communication skills and the ability to connect with patients and families from diverse backgrounds.
• Proficient in mental health assessment and therapeutic techniques applicable in hospice settings.

• Experience working with terminally ill patients or in hospice care is preferred.
• Proven track record of providing individual and family therapy, along with palliative care experience.
• Familiarity with electronic health record (EHR) systems and telehealth platforms.

The annual salary for this position ranges from USD 80,000 to 100,000, commensurate with experience and qualifications.
