Key Responsibilities
  • Conduct comprehensive clinical assessments to evaluate the psychological, emotional, and social needs of patients in the hospice setting.

  • Implement therapeutic interventions that utilize solution-focused methodologies, enhancing patients' coping mechanisms during their care journey.
  • Maintain meticulous clinical documentation that complies with all regulatory requirements, ensuring that patient records are accurate and timely.
  • Collaborate closely with other healthcare professionals, including doctors, nurses, and chaplains, to create a cohesive and coordinated care plan for patients and their families.
  • Facilitate case consultations and peer review meetings to discuss challenges, share insights, and enhance the quality of care across the team.
  • Provide education and support to patients and their families, empowering them with the knowledge necessary to navigate their unique circumstances.

Patient Population / Client Focus

As an LCSW in our hospice care environment, you will primarily work with patients facing terminal illnesses and their families. Your role will focus on addressing the emotional and psychological needs of individuals at a vulnerable stage in their lives, providing them with the support, compassion, and resources they require to achieve dignity and comfort in their final days.

Qualifications
  • A valid LCSW license in the state of Virginia is required.
  • A Master's degree in Social Work from an accredited institution.
  • At least two years of clinical experience in social work, focusing on mental health, geriatric care, or hospice settings.
  • Proficiency in solution-based therapy approaches, particularly in contexts related to end-of-life care.
  • Strong communication skills and the ability to work effectively in a collaborative environment.

Preferred Experience
