Gotham Enterprises Ltd is in search of a dedicated and licensed Clinical Social Worker (LCSW) to contribute to our hospice care team in Columbus, Kansas. This full-time role allows you to work remotely while providing vital mental health support to patients and their families during challenging times. • Conduct thorough clinical assessments to identify patient needs, including emotional and psychological challenges.

• Develop detailed, individualized treatment plans that adhere to best practices in mental health care.

• Implement evidence-based therapy modalities, such as Cognitive Behavioral Therapy (CBT) and solution-focused brief therapy, to help patients achieve their mental health goals.

• Maintain accurate and timely clinical documentation using electronic health record systems to track patient progress and outcomes.

• Collaborate with physicians, nurses, and other multidisciplinary team members to ensure comprehensive treatment approaches.

• Provide education and emotional support to both patients and their families, helping them navigate the complexities of end-of-life care.

• Engage in continuous professional development by staying informed about the latest research and advancements in mental health care.

• A Master's degree in Social Work from an accredited institution is required.

• A valid LCSW license in the state of Kansas is mandatory.

• Prior experience working in a hospice, palliative care, or similar healthcare setting.

• Familiarity with electronic health record (EHR) systems for documentation and patient management.

• A proven track record of successful collaboration within a multidisciplinary team.
