Job description

Job Summary

We are seeking an experienced Emergency Department Facility Medical Coder to join our team. In this role, you will review emergency department medical records and accurately assign ICD-10-CM diagnosis codes, CPT/HCPCS procedure codes, modifiers, and facility Evaluation & Management (E/M) levels. The ideal candidate will have strong knowledge of emergency department facility coding guidelines, regulatory compliance, reimbursement methodologies, and coding quality standards.

This position requires attention to detail, critical thinking, and the ability to interpret physician, nursing, and ancillary documentation to ensure accurate code assignment, charge capture, and compliance with coding regulations.

Key Responsibilities
  • Review emergency department medical records and assign accurate ICD-10-CM diagnosis codes, CPT/HCPCS procedure codes, and applicable modifiers.
  • Assign facility Evaluation & Management (E/M) levels in accordance with client-specific emergency department leveling guidelines and facility charging methodologies, including use of the ACEP tool.
  • Analyze documentation from physicians, nursing staff, ancillary departments, and diagnostic services to support complete and accurate coding.
  • Ensure appropriate assignment of procedural services, injections, infusions, and related charges when applicable.
  • Apply official coding guidelines, CMS regulations, National Correct Coding Initiative (NCCI) edits, and payer-specific requirements.
  • Identify documentation deficiencies and communicate coding clarification needs through appropriate channels.
  • Maintain compliance with Medicare, Medicaid, commercial payer, and organizational coding policies.
  • Participate in coding audits, quality reviews, educational programs, and continuous process improvement initiatives.
  • Stay current on coding updates, reimbursement changes, and regulatory requirements impacting emergency department facility coding.
  • Collaborate with coding leadership, auditors, and client stakeholders to resolve coding-related questions and discrepancies.
Required Qualifications
  • Experience coding Emergency Department facility records.
  • Strong knowledge of ICD-10-CM, CPT, and HCPCS coding.
  • Understanding of facility E/M leveling methodologies and emergency department charging guidelines.
  • Knowledge of CMS regulations, payer requirements, and NCCI edits.
  • Experience reviewing complex medical documentation and ensuring accurate charge capture.
  • Strong analytical, organizational, and problem-solving skills.
  • Ability to work independently while meeting productivity and quality standards.
Preferred Qualifications
  • Experience using Epic or other electronic health record (EHR) systems.
  • Familiarity with the ACEP Emergency Department Facility Coding Tool.
  • Professional coding certification such as:

Pay and Benefits

The pay range for this position is $22.00 - $28.00/hr.
