What is the Telehealth Heart Failure Registered Nurse role?

We are looking for a full-time (FT) RN specializing in cardiology and heart failure to remotely monitor and manage patients with heart failure. Bilingual candidates (English and Spanish speaking) are highly preferred. In this role, the RN will track and trend vital signs, virtually assess patients, document and work with the clinical care team to coordinate care. Our RNs will use their excellent clinical and interpersonal skills to work with patients and the members of the clinical care team to provide high standards of clinical care. They will think critically and creatively about what is needed to enhance the patient's care.

The ideal candidate has strong clinical judgment, critical thinking, and interpersonal skills to provide high-quality patient-centered care. They will proactively assess trends in patient data, identify potential concerns, and intervene appropriately to optimize heart failure management. Additionally, they will play a key role in patient education, empowering individuals and their caregivers to manage symptoms effectively and adhere to their care plans.

We are seeking full-time coverage Monday through Friday from 8:00 AM to 4:30 PM PST.

What you'll be doing:

  • Triage patients using protocols to determine what measures can be taken by the nurse and when to escalate to appropriate level of care.
  • Provide a virtual nursing assessment to evaluate patient symptoms, medication adherence, and disease progression.
  • Provide patient education on heart failure medications, adherence, and potential side effects to patients and caregivers
  • Monitor patient vital signs and survey responses, identify changes in condition and document.
  • Document interactions and interventions.
  • Answer, direct, and follow up on incoming calls.
  • Recognize urgent or emergent situations, seek assistance of the medical provider, and initiate appropriate emergency interventions as directed.

What you'll bring:

  • Bachelor's Degree in Nursing.
  • Minimum of 3 years experience in cardiology, heart failure management, or critical care nursing.
  • Strong background in heart failure patient management, including knowledge of GDMT (Guideline-Directed Medical Therapy).
  • Advanced certification highly preferred (CCRN, CHFN, RN-BC)
  • Current license to practice as a Registered Professional Nurse in a compact state and the state of California. Additional licenses preferred: NY, IL, FL, MO, KY, RI. (Must have a CA license)
  • Experience with patient phone triage either in-office or via telemedicine.
  • Ability to learn and proficiently use multiple EMRs and various communication applications, including Google products.
  • Must be comfortable with the virtual use of episodic vital sign monitoring devices.
  • Ability to work from home in a HIPAA compliant environment is required.
  • Fluency in English and Spanish is highly preferred.
  • Some holidays may be required.
