Job description

Shepherd Hospice LLC is seeking a Social Worker to join our team and support patients and families through compassionate, person-centered care in New Albany, MS and surrounding areas.

Position Summary

As a Social Worker, you will provide professional psychosocial support to patients and their support systems, helping navigate the emotional, practical, and community resources associated with serious illness and end-of-life care. You will work collaboratively with an interdisciplinary team to assess needs, develop care plans, and ensure services are coordinated effectively.

This is a full-time remote role based in New Albany, MS, with opportunities to engage with patients and families through ongoing case management, advocacy, and supportive counseling.

Key Responsibilities
  • Conduct psychosocial assessments to identify needs, strengths, risks, and barriers to care, and recommend appropriate interventions.

  • Develop, implement, and update care plans in collaboration with the interdisciplinary team to support patient goals and family needs.

  • Provide counseling, crisis support, and education to help patients and families cope with stressors, changes in health, and care transitions.

  • Coordinate referrals and connect patients with community resources, financial assistance options, and supportive services.

  • Maintain accurate, timely documentation and ensure compliance with applicable care standards and organizational policies.

Required Qualifications
  • Bachelor’s degree in Social Work or a closely related field, with relevant experience in a clinical or community-based setting.

Compensation

Annual salary: $40,000.00 - $72,000.00
