• Conduct thorough clinical assessments to determine the psychosocial needs and concerns of patients and their families.
• Develop and implement individualized treatment plans that incorporate evidence-based therapeutic interventions tailored to each patient's circumstances.
• Maintain accurate and timely clinical documentation while adhering to ethical standards and best practices.
• Actively engage with a multidisciplinary treatment team, contributing to case discussions and treatment strategies to enhance patient care.
• Provide education and emotional support to patients and families, helping them navigate the complexities around end-of-life choices and bereavement.
• Stay current with mental health research and treatment innovations, integrating new approaches to enhance patient care.

In this role, you will work primarily with patients in hospice care, focusing on individuals facing terminal illnesses and their families. Understanding the emotional and psychological challenges faced during this stage of life is crucial. As an LCSW, you will play a pivotal role in assisting patients in finding peace and establishing meaningful connections and closure.

• A Master's degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of Tennessee is mandatory.
• Comprehensive knowledge of solution-focused psychotherapy methods and evidence-based interventions.
• Excellent communication, interpersonal, and problem-solving skills to effectively support patients and their families.

• Experience in hospice care or working with terminally ill patients is strongly preferred, as it brings an understanding of the unique challenges faced in this environment.
• Familiarity with case management tools and electronic health record systems enhances productivity and patient interaction efficiency.
• Previous experience in a multidisciplinary team setting will be beneficial to navigate the complexities of hospice care.
