Key Responsibilities

• Conduct comprehensive assessments to identify patients' social and emotional needs, collaborating with the treatment team to devise individualized intervention strategies.
• Effectively communicate patient needs and progress to the care team to achieve treatment and discharge goals, illustrating your commitment to patient-centered care.
• Develop and implement detailed discharge plans in partnership with multidisciplinary team members, ensuring that each patient transitions smoothly from clinical services.
• Maintain accurate and timely documentation within the electronic health record, ensuring compliance with state regulations and organizational policies.
• Engage in ongoing professional development, participating in skills training and competency enhancement programs to advance clinical capabilities.
• Assist in various special projects and contribute to organizational initiatives aimed at improving patient services and clinical outcomes.

Qualifications

Preferred Experience

Compensation and Benefits
