As a Licensed Clinical Social Worker, your day-to-day responsibilities will include:

• Conducting comprehensive psychosocial assessments through interviews, observation, and analysis of medical records to determine patients' emotional, social, and environmental factors affecting their recovery.
• Providing individual and family counseling to support patients coping with adjustment issues and emotional well-being throughout their rehabilitation process.
• Collaborating closely with rehabilitation team members to create and implement customized treatment plans that address the psychosocial needs of each patient.
• Assisting patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance, to empower them during their recovery.
• Participating in discharge planning and facilitating smooth transitions for patients moving back to their homes or other care settings, ensuring continuity of care.

You will be working with a diverse patient population that includes individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Your role is crucial in addressing the emotional and social challenges that accompany their physical rehabilitation, allowing patients to reintegrate successfully into their communities with enhanced levels of independence.

The ideal candidate for this role will possess:

• A Master’s degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• A strong understanding of the psychosocial aspects of rehabilitation, including adjustment issues, grief, and coping mechanisms.
• Excellent communication and interpersonal skills to effectively engage with patients and their families.

While experience in a rehabilitation or medical setting is preferred, it is not mandatory. We value a strong foundation in clinical practice, especially in roles involving direct patient interaction.
