What is a remote home health?

A Remote Home Health job involves providing healthcare services to patients in their homes using telehealth technology and digital communication tools. Professionals in this field, such as nurses, therapists, or care coordinators, assess patient needs, monitor health conditions, and offer guidance remotely. This role helps improve patient care while reducing the need for in-person visits, making healthcare more accessible and efficient. Responsibilities may include virtual appointments, coordinating in-home care, and assisting with medication management.

What are the typical daily responsibilities for someone working in remote home health?

In a Remote Home Health position, daily responsibilities often include conducting virtual patient assessments, reviewing patient health data, managing care plans, and coordinating with other healthcare professionals to ensure continuity of care. You might also spend time documenting notes in EHR systems, providing patient education or support via phone or video calls, and triaging potential health issues. Working remotely requires excellent time management and organization, as you'll need to balance multiple cases while keeping accurate records. Many professionals also participate in regular virtual team meetings and ongoing training to stay updated on best practices.

What are the key skills and qualifications needed to thrive in remote home health, and why are they important?

To thrive in a Remote Home Health role, you need strong clinical knowledge, experience in patient care, and relevant licensure such as RN or LPN credentials. Familiarity with telehealth platforms, electronic health records (EHRs), and remote monitoring tools is essential. Exceptional communication, problem-solving skills, and self-motivation make candidates stand out. These abilities ensure that patients receive high-quality care at home while maintaining seamless coordination and meeting healthcare standards in a remote environment.

Job description

About Belle

Belle is a fast growing engagement company that is revolutionizing home healthcare - and it all begins with the feet. With 25M+ U.S. seniors no longer able to see or reach their feet, a lack of self care and mobility challenges cost the health insurance plans $38B+ in preventable medical spending every year.

Belle trains and manages a network of nail technicians or "Belle Technicians" who provide in-home foot care (aka medical pedicures). Belle uses cutting edge data science to identify those most in need on behalf of health plans and deploys its services accordingly. While in the home, Belle Technicians complete assessments and screenings, often being the first to identify emerging issues below and above the knee. As these issues arise, a team of remote nurses coordinate care with other healthcare providers - preventing serious and costly episodes.

Our mission: Bring Joy to Healthcare. Join us if our cause inspires you!

About this Role

Belle is seeking a compassionate and clinically skilled Nurse Practitioner (NP) to provide remote telehealth support for Medicare beneficiaries enrolled in our Chronic Care Management (CCM) program. This role focuses on responding to urgent clinical escalations from our nursing team, with the majority of calls involving poorly controlled hypertension, medication concerns, and other non-life-threatening acute issues.

As a Telehealth Provider, you will assess members remotely, determine the appropriate level of care, provide evidence-based treatment recommendations, and collaborate closely with nurses, Community Health Workers, primary care providers, and care coordinators to help keep members healthy and out of the emergency department whenever clinically appropriate.

This is a fully remote position supporting a fast-growing, mission-driven organization dedicated to bringing joy to healthcare.

Responsibilities
  • Conduct telehealth evaluations for Medicare beneficiaries experiencing urgent, non-emergent clinical concerns
  • Evaluate symptoms, review medical histories, and determine appropriate interventions.
  • Determine when escalation to the member's primary care provider, emergency department, or 911 is clinically indicated
  • Document all encounters accurately and timely in the electronic medical record.
  • Assist in reducing avoidable emergency department visits and hospitalizations through proactive clinical intervention.
  • Provide patient education on disease management, medications, lifestyle modifications, and symptom monitoring.
  • Promote medication adherence and self-management of chronic conditions.
  • Provide clinical guidance and education to nursing staff on appropriate escalation pathways.
  • Participate in clinical case reviews and quality improvement initiatives.

Requirements

  • Active, unrestricted Nurse Practitioner license.
  • National certification appropriate to specialty.
  • Minimum of 3 years of clinical practice experience, preferably in primary care, internal medicine, family medicine, geriatrics, urgent care, or chronic disease management.
  • Experience managing hypertension and common chronic conditions, including diabetes, heart failure, COPD, and chronic kidney disease.
  • Prior telehealth or virtual care experience.
  • Strong clinical assessment and decision-making skills.
  • Excellent verbal communication and patient education abilities.
  • Familiarity with eClinicalWorks (eCW) or similar EMRs.

Benefits

  • Competitive compensation based on experience
  • Health, Dental, and Vision Insurance Benefits
  • 401k
  • PTO, Sick, Wellness leave, and Paid Holidays
  • Opportunity for significant career growth and expansion of responsibilities
  • Ability to reshape an industry and protect lives
