Position Summary:
Requires timely and appropriate care interventions and interdisciplinary communication
to enhance patient satisfaction. Ensures quality documentation and outcomes.

Essential Job Duties and Responsibilities:
• Demonstrate critical thinking to advocate for patient safety, adherence to
company policies, and conflict resolution.
• Review data entry in medical record system to ensure accuracy for billing and
compliance.
• Conduct patient admission assessments, care planning, goal setting, evaluation,
coordination of care, emergency/safety plans as needed per manager direction.
• Ensure the development and implementation of the individualized plan of care.
• Assist with HHCAHPS survey analysis and implementation of quality
improvement activities.
• Understand and comply with policy and procedures set forth by AHC.
• Monitors operations to ensure compliance with county, state and federal
regulations affecting home health care.
• Additional tasks, as assigned by manager.

Education Requirements:

  • Must be licensed as a Registered Nurse (RN) in Minnesota.

Additional Requirements:
• Working knowledge of Microsoft Office suite.
• Excellent oral and written communication skills.
• Ability to read, write and speak fluently in English.
• Education and experience in the scope of services offered by the agency.
• Two years of home care experience including OASIS data collection, preferred.

•Maintain confidentiality in accordance with applicable laws and rules.
• Provide four week written notice prior to termination of employment.
