Responsibilities:

The Northwest Houston Licensed Vocational Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. Must perform 30 visits per week to qualify for FT per diem.


Essential Duties and Responsibilities

  • Under the direction of the RN, assist in identifying the patients physical, psychosocial, and environmental needs. Provide care utilizing infection control measures that protect both staff and patient (OSHA). Records clinical documentation according to accepted regulatory, corporate and professional guidelines. Participates in interdisciplinary communication to ensure that patients needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress. Participates in in-service training programs. Complies with state regulations and national standards
  • Responsible for assuring patient safety.
  • Provides verbal or written patient status updates to supervising RN prior to reassessment, supervisory and/or discharge visits, and as indicated by patient response to treatment.
  • Participates in weekly case conferences.
  • Records treatments administered in medical record.
  • Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.


Qualifications:

Required Knowledge, Skills and Experience


  • Active LVN license in the State of Texas
  • Must live or be willing to work in North Houston
  • Ability to react decisively and quickly in emergencies
  • Knowledge of medications effects, indications, and contraindications
  • Experience with multi-tasking
  • Proficiency in using electronic health records (EHR) system
  • Ability to communicate effectively with patients and their family members, and facility staff
  • Knowledge of basic principles, practices and techniques in home healthcare
  • Ability to read and interpret written information, and write clearly and informatively
  • Ability to speak clearly and persuasively in positive or negative situations, including listening and getting clarifications, and responding well to questions


Preferred Knowledge, Skills and Experience

  • Active LPN/LVN license in the state where patients are located
  • 2 years of experience as an LPN or LVN
  • Familiarity with Medicare and Medicaid insurances and programs
  • Familiarity with home health and hospice
  • Experience in Kinnser/Wellsky (EHR)


Other job duties include but not limited to:

  • Full patient assessment (vitals, etc.)
  • IV therapy
  • Wound care and vac
  • Trachea care, if necessary
  • Peg/G-tube
  • Other duties as assigned
