Baptist Health is looking to add a Registered Nurse to our Hospital at Home team.
Baptist Health is opening an innovative Hospital at Home program that allows patients to complete their hospital stay at home. Patients that will be part of this program are provided with remote patient monitoring (RPM), technology kits, and communication devices. A 24-hour command center staffed by nurses monitors patients virtually, and contracted personnel provide in-home care when required. This program represents a transformational initiative in healthcare delivery.
As the Registered Nurse, you will provide for patient needs as recognized through use of the nursing process (assessment, planning, implementation, evaluation) and the mission statement. You will organize and supervise care provided by supportive personnel, coordinate care provided by ancillary services. Including patients and significant others (SO)/families in developing an individualized plan of care.
Shift Details: PRN
Location: Hospital At Home Contact Center located in Downtown, Jacksonville
Outpatient Stroke Clinic Navigator Registered Nurse Job Responsibilities:
  • Education of patients/SO/families to their disease process and plan of care.
  • Serving as patient advocate
  • Providing care to patients requiring treatment of any medical, surgical, or other condition, including chronic medical and minor injuries or illnesses.

Education & Credential Requirements:
  • ASN Required
  • Basic Life Support (BLS) Required
  • Licensed Registered Nurse Required
