Position Summary

Are you looking for a change of pace from bedside nursing while still making a meaningful impact? The Palms Home Care is seeking an experienced Registered Nurse (RN) to join our team in a remote After-Hours & Weekend Clinical Administrator On-Call role.

This flexible remote position supports our home health branches across Central and Southwest Florida, providing clinical and operational support after normal business hours. You'll collaborate with field clinicians, patients, referral sources, and leadership to ensure seamless patient care, assist with after-hours referrals, coordinate scheduling, and provide administrative clinical support.

Schedule:

  • Monday – Friday: 5:00 PM – 8:00 PM

  • Saturday & Sunday: 8:00 AM – 8:00 PM

  • Flexible scheduling available within these coverage hours.

Responsibilities
  • Serve as the after-hours clinical administrator for multiple home health branches.

  • Support field clinicians with clinical questions and operational needs.

  • Coordinate after-hours and weekend referrals, admissions, and scheduling.

  • Assist with Start of Care (SOC) coordination and clinician assignments.

  • Collaborate with Clinical Managers and Directors to ensure continuity of patient care.

  • Communicate with patients, physicians, hospitals, and referral sources as needed.

  • Review referrals and ensure timely follow-up and appropriate patient placement.

  • Document after-hours activities accurately within the EMR.

  • Escalate urgent clinical concerns to leadership when appropriate.

  • Maintain HIPAA compliance and provide exceptional customer service.

Qualifications
  • Active Florida Registered Nurse (RN) license in good standing.

  • Home Health experience required.

  • Must reside in Florida

  • Strong clinical assessment and critical thinking skills.

  • Previous experience in home health case management, clinical management, intake, or scheduling preferred.

  • Experience with WellSky or comparable EMR preferred.

  • Excellent communication, organization, and multitasking skills.

  • Comfortable working independently in a remote environment.

  • Reliable internet connection and dedicated home workspace.
