• Conduct thorough clinical assessments to evaluate the psychosocial status of patients and families.
• Create and implement individualized treatment plans that address specific needs and goals.
• Utilize evidence-based therapeutic interventions tailored to the hospice care context, including grief counseling and crisis management.
• Document all clinical interactions and maintain accurate patient records, ensuring compliance with HIPAA and organizational policies.
• Collaborate with a multidisciplinary team of physicians, nurses, and other healthcare providers to coordinate care effectively.
• Provide education and emotional support to patients and their families, helping them navigate the complexities of end-of-life care.
• Engage in ongoing professional development and engage with the latest research related to hospice care and mental health.

As an LCSW in our hospice center, you will primarily serve patients who are facing terminal illnesses, along with their families. Your work will focus on addressing the emotional, psychological, and social aspects of their care. By fostering open communication and providing empathetic support, you will help patients and their families process their experiences and cope with the challenges of end-of-life decisions.

• A master's degree in Social Work from an accredited institution.
• A valid LCSW license to practice in Georgia.
• Proven expertise in solution-focused psychotherapy and grief counseling techniques.
• Exceptional communication skills, both verbal and written, with the ability to provide empathy and understanding in challenging situations.
• Strong organizational skills to ensure timely and accurate record-keeping.

• Previous experience working in hospice or palliative care settings is highly desirable.
• Familiarity with telehealth systems and online client management tools.
• Experience collaborating with interdisciplinary teams to support comprehensive care.
