Title: Field Case Management Coordinator-{170377} Location: Illinois- must live in/near the target counties listed: · McLean, De Witt, Macon, Champaign, Douglas, Shelby (and surrounding areas) Pay: $27.70/hr. Schedule: Mon-Fri, 8:00am-5:00pm CST (40 hrs./week) Work Setting: Field-based (travel to member homes) + work-from-home between visits Mileage: Reimbursed Overview We’re hiring a Field Case Management Coordinator to support elderly individuals and people with disabilities who are approved for in-home or nursing facility care . This is a field role requiring local travel to members’ homes, with documentation and follow-up completed from home between visits.

Key Responsibilities Conduct comprehensive evaluations of member needs/eligibility using care management tools and data review Coordinate and implement care plan activities and monitor progress Collaborate with case managers, supervisors, Medical Directors, and other programs to remove barriers and improve outcomes Identify and escalate quality-of-care concerns through established channels Use negotiation and motivational interviewing techniques to increase member engagement and support behavior/lifestyle changes Document activities in compliance with regulatory/accreditation guidelines and company policy Required Qualifications Bachelor’s degree in behavioral health or human services (required) Examples: psychology, social work, marriage & family therapy, nursing, counseling, etc. Case management experience 2+ years in behavioral health, social services, or closely related field Managed Care experience High School Diploma/GED Keywords Field Case Management, Care Coordination, Case Manager, Behavioral Health, Human Services, Social Work, Community-Based, Home Visits, Motivational Interviewing, Managed Care, Elderly, Disability Services
