Home Health RN Quality Specialist:

Position Summary:
Requires timely and appropriate care interventions and interdisciplinary communication
to enhance patient satisfaction. Ensures quality documentation and outcomes.

Essential Job Duties and Responsibilities:
โ€ข Demonstrate critical thinking to advocate for patient safety, adherence to
company policies, and conflict resolution.
โ€ข Review data entry in medical record system to ensure accuracy for billing and
compliance.
โ€ข Conduct patient admission assessments, care planning, goal setting, evaluation,
coordination of care, emergency/safety plans as needed per manager direction.
โ€ข Ensure the development and implementation of the individualized plan of care.
โ€ข Assist with HHCAHPS survey analysis and implementation of quality
improvement activities.
โ€ข Understand and comply with policy and procedures set forth by AHC.
โ€ข Monitors operations to ensure compliance with county, state and federal
regulations affecting home health care.
โ€ข Additional tasks, as assigned by manager.
Education Requirements:
  • Must be licensed as a Registered Nurse (RN) in Minnesota.

Additional Requirements:
โ€ข Working knowledge of Microsoft Office suite.
โ€ข Excellent oral and written communication skills.
โ€ข Ability to read, write and speak fluently in English.
โ€ข Education and experience in the scope of services offered by the agency.
โ€ข Two years of home care experience including OASIS data collection, preferred.
โ€ข Maintain confidentiality in accordance with applicable laws and rules.
โ€ข Provide four week written notice prior to termination of employment.
